PREGNANCY IN OLDER WOMAN
These women need careful surveil¬lance throughout their pregnancies by experts. Many of these complications can be avoided or dealt with success¬fully, should they occur. Expert super¬vision, regular prenatal check-up and strict adherence to the doctors advice go a long way towards ensuring an uneventful pregnancy.
HEIGHTENED AWARENESS
Labour seems to be more painful in these women as a result of their heightened awareness of the compli¬cations which may occur. Fear and anxiety as well as less resilience in the muscles of the pelvic floor and vagina, contribute to the increased intensity of the pain and prolonged labour. There is about 25% increase in the duration of labour in these women compared to that of younger women.
However, now that epidural analge¬sia is available, the pain of labour can be completely eliminated. This involves injection of a local anesthetic drug in the spines at intervals through a thin tube placed inside the backbone.
The incidence of instrumental deliv¬ery, such as forceps delivery, is also higher in such women. There is also a definite increase in Caesarean deliver¬ies. The fact that the women and their doctors put a high premium on such a pregnancy contributes significantly to the choice of a Caesarean section.
However, it is pertinent to under¬stand that a Caesarean operation is a fairly straightforward procedure when performed by an experienced gynecologist in a well-equipped hospital.
The 6-week period following deliv¬ery (the puerperium) in older women is not very different from that of younger women except for the fact that breast¬feeding is less successful in the former. It is clear then that it is feasible to conceive and mother a child safely and joyfully, when one is in the 30s. Medical advances in the past few years make us this promise.
A woman in the third decade of her life should make up her mind quickly regarding whether or not she wants to embark upon the challenges of moth¬erhood. She should not let valuable years pass by simply sitting on the fence with fear and apprehension.
